MPH Community Fund - help us and fill in our survey

As the Community Fund approaches its third year, the MPH Community Development team have launched a major review of the Fund, its objectives, processes and the programmes and support offered.

The review will inform the forward plan for the Fund for 2012 -2015.

Whether you have already been involved with the Community Fund in any way - maybe currently in receipt of grant funding from us or are in the middle of our current application process? Or you have applied for funding and were not successful or just expressed an interest in the fund but decided not to apply, we would really like to encourage you be involved in this review.

Please help inform our future plans for the Fund by filling in our questionnaire which can be accessed by clicking on the following link MPH Community Fund survey .
If you would prefer to complete a paper questionnaire please let us know and we will post one to you.
Completed questionnaires must be returned to us by Wednesday 18th January 2012
